挑戰者接到邀請後，可以選擇在24小時之內拿起裝滿冰塊的冷水朝自己潑下去，或捐出100美元到ALS基金會，當然也非常歡迎兩者都做，接下來就可以指名3位挑戰者接力。挑戰者們將自己的挑戰影片上傳到網路，透過影片的傳遞，讓這個慈善活動如蝴蝶效應一般擴散出去。
